What can this treatment possibly do?
The visible signs of facial skin aging include surface irregularities, such as mottled red-brown dyschromia and fine wrinkling and also coarser textural changes, such as sagging and wrinkling of facial skin. Indeed, skin laxity is a common sign of photoaging.
Following loss of volume and elasticity in the facial skin, tissues follow gravity in a downward movement – giving the saggy facial skin look.
Treatment with ultrasound energy via the Ulthera system can improve and lift saggy facial skin. It may improve skin laxity (sagginess) seen at the nasolabial fold, jawline, neck and brow.
Ultrasound energy has specific characteristics that may increase its suitability for skin tightening. First, it is popularlyimm believed that energy delivery to the deeper subcutaneous layers of the face, or even the superficial musculoaponeurotic system, is effective in inducing skin tightening.
Further, to the extent that this delivery can be divorced from secondary scatter and absorption in the epidermis and dermis, the risk of inadvertent cutaneous injury can be reduced. Apart from ionizing radiation, ultrasound is the other type of inducible energy that can be delivered arbitrarily deeply into tissue in a selective manner.
Intense ultrasound energy works as follows: an ultrasound field vibrates tissue, thus creating friction
between molecules, which absorb mechanical energy that leads to secondary generation of heat. Overall, selective coagulative change is affected within the focal region of the beam but other tissue
proximal and distal to the focal region of the ultrasound field is preserved.
What medical technology or device does this treatment utilise and how it is done?
Ulthera utilises HIFU (High Intensity Focused Ultrasound) energy, and is a US FDA approved system.
This treatment uses the Ulthera System, which is a device that delivers inducible ultrasound energy to selected focus within the dermis and subcutis causing the generation of heat and selective coagulative changes. The generated heat causes initiation of the tissue repair cascade in which the end result is a tightening effect of the skin.
Results from several studies have led Ulthera to receive the Food and Drug Administration approval for skin lifting, initially for eyebrow lifting in 2009, followed several years later with an approval for skin lifting of the neck and submentum.
Ultrasound energy (which is produced by the Ulthera system) is an energy modality that can be focused and penetrates deeper in the tissue to cause thermal coagulation. Intense focused ultrasound (IFUS) for skin rejuvenation has been shown in recent studies to be safe and effective for skin tightening and lifting.
What are the likely results? Any side effects or downtime?

Ultrasound appears to be a safe and effective modality for facial skin tightening. A single ultrasound treatment of the forehead produced on average brow height elevation of approximately 1.7 to 1.9 mm. This effect was seen in many treated patients and was preserved 3 months after treatment.
In addition, both dermal collagen and elastic fibers were significantly regenerated and increased in number, resulting in thickening of the reticular dermis with no significant change in the epidermis. It can be concluded that it is via this dermal collagen regeneration that the rejuvenation of infraorbital
laxity is achieved.
This microcoagulation is thought to cause gradual tightening of the skin through collagen contraction and remodeling. The onset of collagen denaturation with subsequent tissue contraction by 3 months, and the duration of clinical lifting responses lasting for about 1 year.
Side effects were limited to transient redness and swelling, which are common to all laser, light, and other energy treatments.
This treatment has synergy and can be combined with?
Various energy-delivery devices can also be used in combination with Ulthera to treat several other components of skin aging, including skin wrinkles, laxity, and pigmentation, such as co2 Laser, deep chemical peels and facial dermabrasion.
These methods rely on ablation of the epidermis causing reepithelialization while delivering significant
thermal injury to the dermis sufficient to stimulate a robust wound-healing response with subsequent collagen remodelling and contraction causing decreased skin wrinkles, improvement in skin texture, skin tightening, and improvement in pigmentation.
