What can this treatment possibly do?
This treatment uses the Thermage radiofrequency system to iron out static facial wrinkles, and provides skin tightening to wrinkly skin. Many studies reported that RF is well suited for patients with early signs of aging, with mild to moderate wrinkles.
There are two clinically and biologically distinct aging processes affecting the skin. The first is intrinsic aging, ‘‘the biologic clock,’’ which affects the skin by slow, irreversible tissue degeneration. The second is extrinsic aging, ‘‘photoaging,’’ which was first described as the effects of chronic exposure to the elements, primarily ultraviolet radiation on skin.
The histologic and ultrastructural hallmark of photodamaged skin is the accumulation of elastotic material in the papillary and mid dermis, a process known as solar elastosis.
Collagen, which comprises of more than 80% of the total dry weight of the dermis, becomes disorganized with enhanced breakdown and reduced network formation.6 These alterations contribute to the skin sagging and wrinkling.
What medical technology or device does this treatment utilise and how it is done?
This treatment uses the Thermage system, which utilises Radiofrequency energy. It is an effective and valuable procedure that can be used to tighten and rejuvenate photoaged skin, and contour facial skin laxity. This modality stimulates the repair process, and reverses the clinical, and the histopathological, signs of aging, with the advantage of relatively risk free procedure and with little downtime.
The radiofrequency energy from the Thermage system delivers uniform heat at controlled depth to dermal layers, causing direct collagen contraction and an almost immediate skin tightening. Subsequent remodeling and reorientation of collagen bundles and the formation of freshly produced collagen is achieved over months after treatment.
The medical use of RF is based on an oscillating electrical current that forces collisions between charged molecules and ions, which are then transformed into heat. The dermis is composed of collagen, elastin, and ground substances. RF mediated thermal stimulation of this matrix results in a change in the helical structure of collagen. It is believed that RF thermal stimulation results in a microinflammatory stimulation of fibroblasts, which produces fresh collagen (neocollagenesis) and renewed elastin (neoelastogenesis), as well as other substances to enhance dermal structure.
What are the likely results? Any side effects or downtime?
Clinical results showed noticeable improvement at the end of treatment with the Thermage radiofrequency system; with continued improvement 3 months post treatment. Improvements in skin tightening increased substantially at the end of treatment at 3 months post treatment.
Appearance of facial wrinkles was lesser 3 months post treatment as compared with the end of treatment.
Apart from improvement in skin tightening, improvements from wrinkles and lines can also be seen in nasolabial fold and periorbital wrinkles, 3 months after the treatment session with single treatment and multiple passes.
Multiple treatments with multiple passes could give visible results. For patients who received more than a single treatment, it appeared that subsequent treatment sessions further reduced their wrinkles and facial laxity.
Photoaged skin is associated with a decrease in epidermal thickness with flattening of the rete ridges. Although observed radiofrequency energy targets the dermal layer, striking changes in histologic features of the epidermis were observed; there was also a noticeable increase in epidermal thickness at the end of treatment, and at 3 months post treatment, especially in the granular cell layer. These suggest that proliferation of cells in the epidermis is increased, and perhaps may contribute to the improvement of skin appearance.
Over time, as a thermally mediated healing response, fibroblasts are stimulated by radiofrequency energy to enhance fresh collagen deposition and remodelling, resulting in further collagen tightening, and an overall increase in collagen content.
Wrinkle reduction, by means of radiofrequency thermal heat energy delivered to the dermis, is based on the stimulation of fresh collagen formation. Quantitative evaluation of dermal collagen revealed a significant increase in both type I and III collagens at the end of treatment.
