The stigma of acne scarring is evident to those afflicted. The cruel reality for many, even when acne eventually abates, is the facial disfigurement of acne scars. Others perceive those with facial acne scars as insecure and shy as being less attractive, confident, healthy and successful. It is unsurprising then that acne scarring is associated with embarrassment, self-consciousness, anxiety, and depression.

 

Acne scarring is the result of an alteration of the healing process and it can have deep psychological implications for patients. Scars can involve textural change in the superficial and deep dermis (skin) and it can also be associated with erythema (patchy redness) or pigmentation.

 

What medical technology or device does this treatment utilise and how it is done?

 

Anecdotal experiences and medical investigations have shown that majority of cases of acne scarring cannot be solved by a single mode of treatment. There is no “best” treatment here. Acne scars vary for type and depths. A combination of treatment modes is therefore ideally suited to address all the types of acne scarring which may be present on the face.

 

The treatment techniques and modes we have in our clinic are as follows:-

 

Chemical Peel Exfoliation

 

Chemical Peel exfoliation is obtained by applying chemicals to the skin to destroy the outer damaged layers and accelerate the repair process. We have a variety of different chemical peel agents such as Glycolic Acid (GA), Salicylic Acid (SA), and Trichloroacetic Acid (TCA) which can be used depending on the type of acne scars we see.

TCA is a medium depth peel suitable to treat acne scars, both deep and superficial.

​

Lavieen BB Laser

This is a 1927nm Thulium laser - a sub ablative skin resurfacing laser which is able to resurface acne scars and improve pores with lesser downtime and lower risk of post treatment hyperpigmentation as compared to Fractional CO2 laser. With a single procedure, patients can expect improvement in skin texture, dull skin tone and pigmentary changes, similar to the make-up effect of applying BB cream. With a higher power and longer pulse duration, it can simulate the effects of Fractional CO2 laser with lesser recovery time.

Skin Radiofrequency Microneedling with the INFINI system

 

Skin Radiofrequency Mirconeedling using the Infini system is a dermatological treatment procedure performed with the Infini machine to achieve collagen induction, which is effective in improving depressed acne scarring.

 

The needling of the skin is done by a the Infini tip, which consists of multiple insulated and thin microneedles which are applied onto the skin areas which are affected by acne scars. The skin is punctured in multiple direction in a controlled fashion by the insulated tip of the Infini machine. In addition, the insulated mirconeedles of the Infini machine releases radiofrequency waves to act deeper in the dermis.

 

The insulated microneedles penetrate from 1.5 to 2mm into the skin. Penetration depth can be controlled in a calculated manner by the Infini machine, which allows for customization of treatment. The radiofrequency energy can also be adjusted. Having insulated needles (except for the active tip), also insures maximum thermal injury zones in the targeted zones while allow little to no long term damage to the epidermis.

 

As expected, the skin bleeds for a short time and develops multiple micro bruises in the skin. This damage induces the release of growth factors that stimulate the production of fresh collagen and elastin in the upper dermis. Several studies report that 6 months after collagen-induction therapy, histology shows a dramatic increase of freshly produced collagen and elastin fibres.

 

Results from Radiofrequency Microneedling generally starts to be seen after about 6 weeks, but complete improvement can take at least 3 months to occur, and as the deposition of freshly produced collagen takes place slowly, the skin texture will continue to improve over a 12-month period. Majority of individuals will require around three treatments approximately 4 weeks apart.

 

Ablative Carbon Dioxide Co2 Laser

 

Ablative Co2 Lasers achiever removal of the damaged acne scar tissue through melting, evaporation, or vaporisation. Co2 Lasers are commonly used ablative lasers for the treatment of acne scars.

 

Carbon Dioxide Co2 Laser delivers heat energy directly to the skin. The laser energy rapidly heats and vaporises the scar tissues, which causes collagen remodelling and heat mediated tissue contraction. After treatment, fibroblast depositing freshly produced collagen and elastin fibres can be seen, with the reduction of the appearance of acne scars.

 

Subcision

 

Subcision is a stand-alone treatment for depressed acne scars and wrinkles designed to address the underlying pathophysiology of rolling acne scars, Despite their superficial appearance, rolling scars result from deep fibrous attachments gathering the epidermis to the subcutis. Subcison is designed to cut these fibrous bands while causing minimal damage to the overlying skin.

 

Typically, this technique results in the elevation of the depressed scar to the level surrounding skin. In the weeks following subcision, additional augmentation of the depressed defect is typically observed. This subsequent elevation is thought to result from trauma caused during the procedure which initiates wound-healing response cumulating in the deposition of freshly produced connective tissue beneath the scar surface.

 

The main advantage of subcison is that it has the potential to produce long term improvement in the appearance of rolling scars while causing minimal injury to overlying skin.

 

What are the likely results? Any side effects or downtime?

 

An improvement of facial appearance with a reduction of acne scars should be seen. It is however important that treatment approaches for acne scarring should be individualised and primarily determined by the morphological features of each patient’s scar.

 

All the various treatments above are generally safe and well tolerated. Some bleeding, mild swelling, redness and bruising are to be expected.
